Choosing the Right Computer System for your Medical Practice

Most of the physicians I have met are just too afraid to change their existing computer or even to upgrade their existing medical billing software. Their staff is resistant to transition or even learning a new system!

Making a decision on what computer hardware to choose is very critical to your practice and for your compliancy. The good news is, computers nowadays are incredibly affordable! You can now get a complete desktop with monitor with as low as $450.00 Most of these computers are windows-based operating system. Your existing medical billing software will definitely be compatible on any of these Windows operating systems. Your medical billing software vendor might just run few updates and patches for full compatibility with your new computers or server or even on your old computers! The same thing if you are upgrading your billing software. The bottom line is, you are about to make a decision to make your practice productive, efficient and compliant with HIPAA rules and guidelines.

Let's start with, if you are a small practice with 2 computers and 1 printer. Well, having a dedicated server for your billing system is not yet a good option.

(1) Buying a computer running on Windows XP Professional is better than the one that runs on XP Home Edition. XP Professional is really intended for business purposes which runs database and other office applications software.

(2) Share your computer via a switch or a router by networking the two computers. Why share? sharing files and documents is very productive. In other words, you can access files from any of these computers.

(3) Share your printer, connect one printer locally and share it on the network.

If you are a bigger practice with more than 3 computers, 2 printers and at least 5-10 staff (including the providers). Then you are better off to go for a dedicated server or a stand-alone server. What does this mean? ofcourse, can you imagine at least 7 people simultaneously accessing the same medical billing software (setting up appointments, taking information for new patients, billers posting payments and the providers putting in notes on the patients), this will definitely slow down your network if you don't have a stand-alone server.

That server is best to run on Windows Server 2003 (Business Edition) or on Windows 2000 Server (Advance Edition). All these 3-8 computers will share resources by networking them using a switch (or a high-end hub). Printers will also be shared on the network, all users may print from any of the network printers from any of the computers.

Consider the following hardware specifications for your server and workstations:

(1) Processor (I prefer Intel-based processors)

(2) Memory (at least 512MB)

(3) Hard Disk Storage - Server (at least 250GB, this is just my suggestion!)

(4) Hard Disk Storage - Workstations ( at least 40GB)

(5) Tape Back Up (you have to have a daily back up of your files!, internal tape back up is more convenient than external back up drive)

(6) Network Interface Card

How about the warranty? most of the vendors sell warranty up to 3 years or even 5 years for parts and repairs. This will cost you another $150-$300 for warranty. I strongly encourage you to get that extended warranty because you never know.

Call your credit card company immediately. Rarely, will you be liable for charges over $50, and many credit card companies wave the first $50 in the case of fraud. Check with your bank, you will usually be given a choice to close your account and reopen a new one or some banks will put a "watch" on your account and you will have to log into your account and manually approve every charge and check before it is cleared. This usually goes on for a period of 30-60 days. The latter isn't the best choice as it is labor intensive to do and your account isn't protected from fraud as it would be if you simply closed it out and reopened a new account.

After you have done all the initial damage control. The next thing to do when you realize that you are a victim of identity theft is to notify the credit bureaus. Report the situation to the three major credit reporting companies- Equifax, Experian, and TransUnion. You can do it online or in writing, and you must tell these companies that your identifying information has been stolen and is being used by another person fraudulently in your name. Ask them to flag your file with a fraud alert, and ask that all creditors call you before extending credit in your name. A consumer statement will be put on your file and it will alleviate your fears of bogus accounts being opened for about 3 months.

Due to provisions of the Fair Credit Reporting Act, you can only place an initial fraud alert for 90 days. The credit bureaus will then send you a notice displaying your rights as an identity theft victim. As soon you receive this, write to each of the three credit bureaus to request two things; An extension of the fraud alert to seven years, and a free copy of your credit report. Remember to include an identity theft report when you send your letter in order to authorize the seven year alert. Whenever you communicate with the credit companies, you should refer to the number given to your credit report and always use verified return receipt mail. Also, make sure you save all credit reports and related information for your records.

As soon as you get your three credit reports, carefully examine each one. Report all fraudulent and inaccurate information in writing to the credit bureaus, as well as to the credit issuers, following the instructions that are provided with the credit reports. As soon as you notify the credit bureaus about the fraudulent information, the bureau is required to strike that information from future reports. The bureau will also notify the credit grantor of the fraudulent account(s). If it isn't included in the report, ask the credit bureaus for the contact information of the credit grantors.

Also, tell the credit bureaus in writing that you want them to remove all inquiries that have been created due to fraudulent access. You should also ask the bureaus to contact anyone who has received your credit report in the last six months so they can be informed about the fraudulent and inaccurate information. Numerous inquiries can lower your credit score, so work diligently on getting those inquiries removed from your personal credit report.

Though these measures should set you back on track and protect your credit from the fraudulent information, they may not completely stop it from happening again. Because of this, be sure to monitor your credit reports, there are many online services that will help you monitor your credit report. We recommend Experian Triple Advantage. Alternative vs Conventional Medecine

I continue to be blown away by the absolute miracle that I am! Wait a minute, that's not my head swelling all over the page. I'm referring to how awesome a miracle my body is. I've had mine for just over 55 years now and it never fails to amaze me with its ability to take care of itself. Considering what I've put this body through and the amount of abuse I've subjected it to, mine continues to exceed my very best expectations.

I've studied and used "Alternative" or "Natural" healing techniques for a little over half my life now and I'm a stand for its effectiveness. I've, also, had those times when my body needed some drastic assistance and I go to a conventional doctor for those times. For such drastic situations, Conventional Medicine is just what the Doctor ordered.

First of all, I find the very terms we use to describe these two approaches to medicine to be turned around. We call natural medicine "Alternative" when it precedes AMA type medicine by, at least, a few thousand years and we call the newer form of medicine "Conventional". How twisted is that?

But I digress. I have always been amused by the sibling rivalry, and sometimes out and out hatred, between these two approaches to healing when they have so much to compliment each other. What a tremendous waste.

I blame the AMA and the FDA for most of the conflict, however! Before there was an American Medical Association or a FDA, healers were often Barbers with a sideline interest in general health and before that there was only herbal/natural healing methods usually dispensed by a tribe member or midwife with a gift for helping people.

Then, as humans will do, groups of these healers would gather and compare notes. That led to a desire to organize and format all that knowledge so that all of it would be available to everyone in the group. Presto, the American Medical Association (1847). It was then decided that a uniform set of rules and procedures were needed and that lead to the concept of uniform formal study of medicine and various licenses to practice it. Viola! Doctors and Nurses.

As I see it, the fly in the ointment came when companies formed to produce combinations of herbs, barks, leaves, roots and Chemistry and marketed them to the public in the form of "Snake Oil" and to Doctors & Nurses in the form of "Preparations" which saved them time and energy.

The most famous of these "Snake Oil" remedies is what we now call Coca-Cola, which up until the 1950's contained a concentration of Cocaine and was marketed as a cure all for whatever might ail you. It made people feel better,,, imagine that! Dr. Pepper was another. The 3 numbers on Dr Pepper bottles produced up until the 80's referred to the times of day the remedy should be taken.

With that kind of medicine being bought sold all over the country it was no surprise that some supervision of drug companies was in order. Viola! The Food & Drug Administration (1862) popped up to fill the need. Eventually the drug producers, drug regulators and drug prescribers became so cozy with one another that Natural medical preparations got pretty much pushed aside by the medical establishment as "Old School" and lumped into the same category as the old "Snake Oils".

The 1960's brought in a re-awakening of interest in nature and its healing properties and that's when "Alternative" medicine came under fierce political opposition from the medical "Establishment". By then the medical establishment had too much to lose to allow people to regain control over their own health care.

But, back to my original point; both approaches to medicine and healing have their strengths and their weaknesses. However, the two together make a powerful combination to support the whole healthy human. For preventive care and maintenance, "Natural" or "Alternative" medicines have no equal. For critical or catastrophic health conditions, "Conventional" medicine has no equal.

I long for the day when these to siblings just admit that they are both unequalled in their own special area of care and start working together for the good they have in common towards the proper care and maintenance of the Whole Human Being.

Choosing Your HDTV Television

To take full advantage of the great number of HDTV stations you can get with your satellite system, you will want a television with a breathtaking picture and mind blowing sound. When shopping for a television today, you have many more options than just a few years ago. From several hundred dollars, to a price similar to that of a new Corvette, you can choose the television to fit your budget and lifestyle.

If you want to watch satellite programming you will need a tv that is at least HDTV ready. Your satellite service provider will supply you with an HDTV tuner. Even if you will not be watching much HDTV programming, there is still a huge benefit to watching a digital television. All HDTV and HDTV ready tv's contain upconversion circuitry that cleans up the picture with even non HDTV signals.

If you watch a lot of movies or dvd's, you might want to purchase a widescreen television. A widescreen tv has a 16:9 aspect ratio screen, as opposed to the traditional television screen with a 4:3 aspect ratio. You can enjoy watching movies on a screen shaped like a movie theater screen, and view non digital programming in a side cropped view, or stretch the picture to varying degrees to fill the screen. All HDTV programming uses the 16:9 aspect ratio.

The primary drawbacks of tube tv's are their immense weight (compared to newer technology), and the considerable depth of the sets themselves, both severely limiting your installation choices. Also, the largest tube television available is 36", so if you need a larger picture, you might want to step up to flat-panel television.

LCD and Plasma screens are the two options available in flat-panel televisions. Both are incredibly thin and lightweight, and can be wall mounted if you choose to do so. An LCD (liquid crystal display) contains thousands of crystals sandwiched between two glass or plastic plates, which are acted upon by electrical current, creating images on the screen. A plasma screen consists of thousands of pixel cells filled with gases that excite phosphors, producing light, and thereby your picture.

The weight of an LCD and a plasma tv are comparable, with the LCD models being slightly lighter in the 40" range. As the screen size approaches 60", the weight of the LCD overtakes that of the plasma screen. The display panel tends to last longer with the LCD than with the plasma models. LCD televisions are not vulnerable to burn-in, also known as image shadowing or ghosting, like plasma screens. Burn-in is essentially burned, or worn out pixels in a plasma screen that have been damaged over a period of time and retain color information from static images like network logos, etc... Plasma tv's tend to have more accurate color reproduction than LCD models, and are currently less expensive than a comparable LCD. While the largest LCD tv at this writing is 65", that is likely to change quickly as the technology evolves with the latest innovations in the computer monitor industry. The largest plasma screen television currently produced is the Panasonic TH-103PZ600U103", which boasts a massive 103" screen, and is expected to carry a whopping retail price of about $70,000 when is hits US markets in late 2006.

Remember the old, bulky rear-projection televisions that were almost a yard deep and weighed nearly as much as a VW Bug? Todays digital big screen tv's take up much less space, and a 60" television can weigh under 100 lbs. Digital big screens are capable of brightness that is unmatched by the older CRT televisions, and do not suffer burn-in like the plasma tv's. These digital big-screens also contains a consumer replaceable lamp that restores picture quality to its original brilliance. Prices for these sets are very reasonable, usually one half to two thirds the price of a similar sized plasma or LCD television. One disadvantage of the big screen tv's is still size; even with technological advancements, these models take more room for installation than the flat screen televisions, and are not practical for wall mounting. The ideal viewing angle is only achieved straight on, and in a seated position.

For a true movie theater experience at home, a front projection tv may be a good choice. This is a two piece system consisting of a projector and a screen. They offer picture sizes up to 300", and do not include any type of tv tuner, instead displaying signals from a satellite receiver or cable box. The best viewing experience with a front projection setup requires a fairly dark environment, and is definitely not for everyone. However, for cinema lovers, there is no better way to bring your favorite film to life. Pricing for good projectors starts at just under $1000, and screen prices vary according to size, with most models falling somewhere between $300 and $1500.

Whether you watch mainly sporting events, movies, or dvd's, a modern television can give new life to your favorite programming. As technology advances at an astonishing pace, prices for larger and more sophisticated television sets continue to come down. If you do some shopping online and find that you can't quite afford the tv that you want, save money for five to six months; no matter how much or how little you are able to save, chances are you will be able to afford the set you want as prices come down. Getting your first pair of pointe shoes is a memory not soon forgotten. From the first blisters to the spotlight at the recital, your shoes will carry you to fulfill your dancing dreams.

It is important to have your pointe shoes fitted by someone who understands the shoes purpose and the dancers bodys demands. Be certain to ask about the credentials of the person doing your fitting, especially if you have concerns about their understanding of your needs.

When you go in for your fitting, bring thin socks or tights to wear in order to keep the shoes clean and create an accurate fit. Ask your teacher if she has any shoe preferences and any padding suggestions. Understand the demands that will be on your shoes by knowing the number of classes per week that you will be taking on pointe and the duration of those classes. Call ahead for an appointment whenever possible to insure individualized service.

Once you find the shoes you like, test them. Take them off, walk around for a few minutes, and then try them on again. If they still feel right to you and look right to your teacher or fitting professional, chances are you have found your best starting shoe.

Challenges of Online Education

Enrollment in online education is at an all time high, according to a recent study by the Sloan Consortium. Based on responses from more than 2,200 colleges and universities across the U.S., the study reveals that 3.2 million students took at least one online course in the fall of 2005, an increase of 850,000 more than the year before. The report is available at www.sloan-c.org/publications/survey/index.asp.

With the rise in popularity of e-learning comes some unique challenges not extant with more traditional classrooms. Students need to be aware of the differences between online and traditional classes and be prepared to make the necessary adjustments. Significant challenges faced by online students include the following:

- Technical problems. Computer hardware and peripheral devices such as printers, modems and routers are not indestructible; as with any other man-made device, they can break down or malfunction at the most inopportune time. In addition to equipment failure, an older computer may be incompatible with some software required for the course. Viruses, trojans, spyware and other malware could infect a computer, causing it to run poorly or not at all. A slow internet connection might cause problems logging into class, retrieving and submitting assignments, and posting to threaded discussions. Any of these problems could cause a great deal of frustration for the student trying to complete course assignments. Therefore, it is important for the student to have access to a backup computer to use in case of equipment malfunction.

- Staying motivated. Going to school always requires a certain level of motivation from a student regardless of whether its a "bricks-and-mortar" institution or online class. In a traditional classroom setting, a student has live, face-to-face interaction with the instructor and fellow students -- a camaraderie that may help boost enthusiasm and motivation for the course. In an internet-based class however, there is no live contact with instructor and classmates -- the student works alone in a virtual environment. This lack of live contact may lead to a feeling of isolation, and make it difficult for a student without a high internal commitment or personal drive to stay motivated during the course.

- Self-discipline. As with motivation, a student needs good self-discipline to succeed online. The freedom and flexibility of e-learning is what's so attractive to many people. There's no obligation to login to class at any particular time or place -- you can go to class and do your assignments whenever it's convenient for you. However, this same freedom can lead to procrastination, and cause the unwary or undisciplined student to fall behind in his assignments. Once behind, it can be difficult to catch up.

- It's difficult to improve oral communication skills. There are many classes such as speech, drama, debate, teacher education and so forth that require the student to make oral presentations in front of a group of people. With online courses this is not practical, as the classroom is virtual, and the students are not all gathered together at one time in one room. A student wishing to improve oral communication skills may need to take these classes in a traditional classroom course.

- Classes with lab or hands-on requirements may not be available online. If a lab course cannot be simulated or practically offered online, a student will need to take the course at a local college campus or other location with the necessary facilities for completing the course work.

Students considering taking online courses need to be aware of these and other challenges, and be prepared to meet them in order to be successful online. If any challenge seems insurmountable, the student should reconsider the online option and perhaps take a traditional ground-based course instead.

What's For Dinner?

How many times have heard that or asked that or asked yourself the same question? If youre anything like me, I got so tired of making the same old things week after week. Cooking became a chore. I was as bored with my cooking as my family was. Cooking was not on my list of favorite things to do.

Going out for dinner is always nice. But boy, has it become expensive! I love to eat good food, but just hated to cook it. I also got tired of spending a fortune for it. That was when I found the most amazing cook book. Now my kids are even chipping in with the cooking. They cant wait to taste the mouth watering food we make. We all get together to try new recipes. Its a great way for my kids to learn how to cook, too.

Have you ever wondered how to make Kentucky Fried Chicken? Now be honest, havent you always wondered what their secret recipe is? I sure have. Now I know. How about Apple Bees Baby back Ribs? Or Olive Gardens Fettuccine Alfredo with bread sticks? One of my favorites is Red lobsters Shrimp Diablo. We used to make a couple of trips a week at least, to Taco Bell. Now we make their Nachos and Buurito Supremes ourselves. We never run out of new recipes to make and they taste just like they do in the restaurants. The only difference is, we are having much more fun making them ourselves and saving a bundle of money to boot!

If you would like to learn how to cook amazing dishes at a fraction of the cost, its not that hard to do at all. Your friends and family will think you are the best cook ever. Believe me, I know. All of my friends ask me for the recipes and say their husbands are amazed with my cooking. They want me to be their cook. I just tell them their wives can cook the same foods just as easy as I can. Or, if you want to, you can keep your recipe book a secret and have everyone totally amazed with your cooking. Cooking can become a great hobby instead of a drag.

Here we highlight what we think are the best ways to push past generational barriers and build strong intergenerational relationships. We've divided the strategies into mindset tips (how to approach cross-generational differences) and practical tips (the small things you can do during a conversation to improve your communication).

Mindset Strategies

Approach with Interest. Approach generational differences with interest, not fear or negativity. Take interest in the interests of others. You can learn fascinating things about other people if you choose to do so.

Take a Learning Orientation. The value of difference is that you can learn from each other. If someone from another generation has specific skills that you don't (say, an appreciation for looking at history and tradition, or the ability to use email effectively), consider setting up a skill exchange. These type of self-development opportunities translate into more opportunities for career advancement as well as more fulfilling and successful workplace environments.

Be Mindful of how your assumptions are influencing your interactions.

Narrow your categories. In your mind, how long do people stay categorized by their generation: "one of the old guys" or "part of that young group"? At what point do they become individuals: Antoine, the man who likes motorcycles, fought in World War II, and thinks Elvis is still the King; Janelle, the woman who runs marathons, loves the city, and is allergic to peanuts? Categorizing people is a natural process that allows us to make sense of the world around us. You'd have a hard time describing an apple without using categories like a type of fruit, sweet or tangy, green or red, Granny Smith or Braeburn. Life is richer and your observations and reflections of people more accurate if you can move away from simple classification and allow for individual variations.

Put yourself in their shoes. Do you know what their day-to-day is like? Do you know what motivates them, excites them, gets them down--or how they want to be treated? Empathize with their situation, needs, and values. You can do this sometimes directly by asking questions and taking an interest in their interests and indirectly by getting involved in some of the traditions and pastimes of another generation. Watch a TV show geared for another generation. Look at a website that focuses on the issues of other generations. Get familiar with music that spans generations like jazz, blues, rock and roll, classic, hip hop, and world music from cultures you are not familiar with. Understanding each others music can help build perspective.

Behavioral Strategies

Be flexible as to the means of your communication (face-to-face, email, etc.)

Avoid generational jargon. Speak in plain terms and avoid idioms that are not widely understood.

Be attentive. Look for signs that you may be misunderstanding each other, whether it is a confused look, an unclear response, or an unintended reaction.

Practice active listening. Turn up your listening dial across generational differences. Listen for clear expressions of different values or outlooks than you have. Seek to understand the individual better by listening carefully to what they say (or don't say).

Show Respect. Most generations have felt they don't get the respect they deserve. Using the strategies above, you can show coworkers that you do respect them, their background, and their outlook on life--and build powerful relationships as a result.

Beginning Pointe Class: What to Expect

Although every teacher is different, the goals of the introductory pointe class are similar no matter where you dance. These goals include strengthening the feet and ankles, breaking in the pointe shoes, and learning proper execution of skills while wearing pointe shoes.

To strengthen the feet, your teacher may instruct in a variety of methods including demi-pointe work, exercises for the feet and ankles, and basic releves at the barre. Instruction may also be given for additional work that can be done outside of class to help you gain strength and skill.

Breaking in pointe shoes is simply the wording used to describe taking the shoe from its initial factory condition to where it is conformed to the shape of your foot. This may include some work with your shoe in hand softening hard spots and bending the shank. I highly suggest waiting for a teachers instruction before bending, softening, or attempting to break in the shoes. Doing so on your own may permanently damage or improperly break in the shoes.

In addition to manually softening the shoes, your feet will serve as your greatest break in tool. Simply doing releves will do much to help the shoes mould to your feet. Again, be careful about doing too much outside of your teachers watchful eye. Doing steps incorrectly in pointe shoes may cause the shoes to break in improperly or lead to an injury.

Lastly, a basic pointe class will include barre work and eventually center work doing the steps you already know. Plies, releves, tendus, jetes, and other basic technical steps will help you learn placement, understand positioning, and gain strength as you develop your pointe skills.

In an introductory pointe class, you may find yourself spending only a few minutes actually dancing in the shoes. Do not be alarmed. It takes dedication and time over the course of many months and years to develop a strong pointe technique. You will gain strength, increase grace, and expand your dancing horizons as you begin pointe. There will be blisters, frustration, and pain, but the beauty of the dance will always make it worthwhile!
